AGE: Each child must be 7 years old by July 31, 2008
and cannot be 15 by July 31, 2008.

Football players will be placed according to Pop Warner ages and weight. (weights and ages)

• Medical Form: This new medical form must be completed by all participants and consists of two sections. Section I can be completed by a parent. Section II must be completed by a state licensed medical professional. This new form is required by National Pop Warner.

2008 Registration Fees:

• $150 for the 1st Child
  (plus $20 for raffle tickets) = $170 for one child in the program
• $85 for the 2nd Child
  (plus $20 per child for raffle tickets) = $275 for two children in the program
• $75 for the 3rd child
  (plus $20 per child for raffle tickets) = $370 for three children in the program
• Maximum of $310/family
  (plus $20 per child for raffle tickets)

Each participant is required to prepay for a $20 Book of Raffle tickets. These pre-paid raffle tickets should be sold and turned in during the first week of practice to your team parent.
